To Qualify, Patients Must:
Have a dementia diagnosis (Alzheimer’s or related)
OR have memory concerns and need evaluation*
Be enrolled in Medicare Parts A & B (not Medicare Advantage)
Live at home or assisted/independent living
Not be enrolled in hospice or PACE
