Who Is Eligible for These Services?

To participate in the program and receive these benefits, patients and caregivers must meet the following criteria.

Patients must:

  • Have a diagnosis of dementia (Alzheimer’s or related), or be experiencing cognitive symptoms and need help getting evaluated.

    Note: AVVACare Concierge can connect you with a physician partner who can help assess cognitive health and next steps if needed.

  • Be enrolled in Medicare Parts A & B (not Medicare Advantage).

  • Live at home or in assisted/independent living.

  • Not be enrolled in hospice or PACE.

Caregivers must:

  • Be a family member or unpaid helper
